Nalbun is an opioid analgesic that works by blocking the transmission of pain signals to the brain, thus lowering pain perception.

• Relief of moderate to severe pain associated with myocardial infarction (MI)
• Supplementing balanced anesthesia
• Preoperative and postoperative analgesia
• Obstetrical analgesia during labor and delivery

✔️ Pharmacology

Nalbuphine Hydrochloride is a synthetic, potent analgesic with effects comparable to morphine. It functions primarily as a kappa agonist and partial mu antagonist. Key pharmacological properties include:

• Receptor Binding: Binds to mu, kappa, and delta receptors, but not sigma receptors.
• Onset of Action: 2 to 3 minutes after intravenous administration; less than 15 minutes after subcutaneous or intramuscular injection.
• Plasma Half-Life: Approximately 5 hours.
• Duration of Analgesic Activity: Ranges from 3 to 6 hours.

✔️ Dosage & Administration of Nalbun Injection

Moderate to Severe Pain:

• Adults: 10-20 mg by intravenous or intramuscular injection for a 70 kg patient, adjusted as required.
• Children: Up to 0.3 mg/kg, repeated once or twice as necessary.

Balanced Anesthesia:

• 0.1-0.2 mg/kg by intravenous or intramuscular injection.

Obstetrical Analgesia (Labor & Delivery):

• 0.3-1 mg/kg by intravenous injection over 10-15 minutes.
• Maintenance dose: 0.25-0.5 mg/kg as required.

Intraoperative Analgesia:

• 0.25-0.5 mg/kg by intravenous injection at 30-minute intervals.

Myocardial Infarction:

• 10-20 mg by slow intravenous injection, repeatable after 30 minutes if necessary.

✔️ Children (18 months to 15 years old):

• 0.2 mg/kg body weight, given preferably by intravenous or intramuscular injection.
• Maintenance doses every 4 to 6 hours, as determined by a physician.

✔️ Drug Warnings for Nalbuphine

• Allergies: Do not take Nalbuphine if you are allergic to any of its contents.
• Severe Conditions: Avoid Nalbuphine if you have acute or severe asthma, respiratory depression, gastrointestinal obstruction, or paralytic ileus.
• Drug Interactions: Do not take Nalbuphine with other opioid analgesics, CNS depressant drugs, or benzodiazepines as it might lead to life-threatening respiratory depression.
• Pre-existing Conditions: Inform your doctor if you have:

• Substance Abuse: Keep your doctor informed if you or anyone in your family has a history of substance abuse as Nalbuphine may cause addiction, abuse, and misuse.

✔️ Contraindications

• Known hypersensitivity to Nalbuphine Hydrochloride.

✔️ Use in Pregnancy & Lactation

• Pregnancy Category B: High and rapid placental transfer. Use only if clearly needed.
• Lactation: Less than 1% of the administered dose is excreted in milk. Caution is advised when administered to nursing mothers.

✔️ Precautions

Exercise caution in patients with:

• Impaired respiration
• Impaired renal or hepatic function
• Biliary tract surgery
• Myocardial infarction
• Hypotension
